Family and Social Services

At The Children's Heart Center we understand the importance of family and its central importance to a child getting and staying well. Social workers and case managers address the needs of patients and families with compassion and promote quality of life while respecting the family's spiritual, emotional and cultural backgrounds.

Qualified professionals in the Case Management/Social Services Department provide education, counseling and advocacy and coordinate community-based support with an emphasis on quality and continuing care.

Assistance is provided for a full spectrum of issues that families may face, including:

• Short/long-term care planning
• Entitlement assistance
• Insurance coverage
• Home health care
• Discharge planning
• Financial services
• Liaison with community agencies
• Medication
• Transportation
• Bereavement counseling
