During all NBIMC floor months (GMF, Oncology, Geriatrics,
Telemetry) interns admit Q 4 nights. PGY-2's admit either Q 4 or
Q 8 nights, depending on their rotation. Five nights per week these
calls are until 8:00 pm, with Night Float relieving floor teams
at 8:00 pm. Friday and Saturday nights are overnight calls, with
house staff leaving by 12:00 p.m. the next day.
All Critical Care rotations involve admitting Q 4
nights overnight. House Staff then are relieved after teaching
rounds the next day, generally by 11:00 am.
The Night Float team consists of two interns and
one PGY-2 who work as a team Sunday through Thursday nights from
8:00 pm till 7:00 am. They are relieved after Morning Report at
8:30 am each day.
Both Interns and PGY-2's participate in the
call schedule for the critical care units during their ambulatory
blocks, on a somewhat variable Q 4 to 8 call basis. They are relieved
after signing-out to the ICU/CCU staff the next day, generally
by 11:00 am. Interns participate in ICU calls and PGY-2's participate
in the CCU call schedule during Ambulatory blocks.
PGY-2's participate in the call schedule for
the CCU during their Neurology consult block, on a somewhat variable
Q 4 to 8 call basis. They are relieved after signing-out to the
CCU staff and rounding with the Neurology attending the next day,
generally by 11:00 am.
During the PGY-3 year calls are divided between coverage
of the Medical Intensive Care Unit (ICU) and coverage as "Medical
Officer of the Day" (MOD), a job which involves major triage and
administrative responsibility for patients admitted to medical
teaching services, as well as oversight of emergency consults and
cardiopulmonary resuscitation efforts. Coverage of these two jobs
is divided as follows:
These are overnight calls, occurring on a Q4 rotation.
These calls are divided between two senior residents assigned to
the ICU, the PGY-3 doing Pulmonary Consults, and the PGY-3 doing
Cardiology and Neurology Consults each block. The residents are
relieved after sign-out rounds and teaching rounds the next morning,
generally by 11:00 a.m.
MOD call: The weekday MOD responsibilities are
divided between a Day-MOD, who works from 8:00 am to 8:00 pm, and
a Night-MOD, who works from 8:00 pm to 8:00 am Monday through Friday.
Saturday and Sunday MOD responsibilities are covered by PGY-3's
on Consult Services other than Pulmonary and Cardiology/Nephrology.
These calls are 24-hour shifts, and generally each PGY-3 does two
or three such shifts during each of their consult months.
