Diabetes Self Management Classes

We offer...

• Certified Diabetes Educators - Nurses and Nutritionists
  
  
• Each session includes:
  4 two hour classes
  1 one hour follow-up class
  
  
• Spouses, parents and significant others included at no additional charge
  
  
• The cost is $55 for each two hour class/$25 for the follow up class. You must attend all of the classes in the session. Make up classes will be offered. A discount will be offered if payment is made in full at the beginning of the session.
  

This program is fee for service. We will provide a receipt of payment for personal submission to insurance carriers for out of network benefit coverage and reimbursement.

Topics covered include:

• Diabetes disease process
• Nutritional management
• Physical activity
• Medications
• Monitoring
• Prevention, detection, and treatment of acute and chronic complications
• Goal setting and problem solving
• Psychosocial adjustment to Diabetes

Classes will be held on Wednesday evenings from 6 to 8 p.m.

For information regarding adult classes please call 973-322-7007.
Classes will be held on Mondays from 10 am to 12 noon.
