JerseyCare Home Health, Clara Maass' licensed, owned and operated
home health care agency, brings the same quality of care that is
provided by the Medical Center into the homes of Essex County residents.
Utilizing a team approach to care, home health staff members carefully
guide and plan each client's home healthcare program so that all
care is integrated for maximum benefit. The team of experienced
nurses and licensed therapists also work with each client's personal
physician to develop an appropriate care program. JCHH developed "Friends
of First Steps" to help ease the transition for new families
from the hospital to the home. "First Steps" offers a
package that provides a home visit by a professional perinatal
nurse and certified home health aide. Visits include instruction
on newborn and postpartum care, lactation support and encouragement.
The service is open to all families, even those who did not deliver
at Clara Maass. JerseyCare Home Health Services is approved for
payment by Medicare, Medicaid and most third-party payors. The
program is Medicare certified and licensed by the New Jersey State
Department of Health and Senior Services.
