For over 20 years, the Van
Dyke Hospice Program of Community Medical Center has been devoted to providing a special kind of care to patients with advanced illness and their families. Van Dyke Hospice is dedicated to offering comfort and support for patients in their final stage along the journey of life. Our program of care includes the latest in symptom and pain management to enhance quality of life. A team of skilled professionals, including RNs, physicians, medical social workers, chaplains, volunteers and the personal care assistance of certified home health aides, care for patients at home or in a nursing home facility. Services include adult and pediatric hospice; pain and symptom management; cardiac special care program; infusion therapy; palliative care; chaplaincy; volunteer training and services; bereavement support for the first 13 months after the loss of a loved one; and memorial services. Jewish Hospice Services is a unique hospice program designed to offer physical, emotional and spiritual care that upholds Jewish culture, traditions and values.
