For more than 25 years, the caring, compassionate professionals
of the nationally-renowned Saint Barnabas Hospice and Palliative
Care Center have provided exceptional physical, emotional
and spiritual support to patients and families during life’s
final stages.

The Center at Monmouth Medical Center offers inpatient and
home care, as well as services in long-term care and assisted
living residences.
Inpatient care is provided on a well-appointed 10-bed unit
on the 6th floor of Monmouth Medical Center. The Unit,
which opened in November 2000, provides symptom management
in an acute care setting for terminally ill patients. Individuals
are cared for in a peaceful home-like environment by the
hospice interdisciplinary team and may be admitted to the
unit directly from their home or from the hospital. Each
patient's physician coordinates with the team regarding medical
management. Visiting hours are not restricted and families
may remain with the patient as desired. The unit features
a dedicated family room and meditation area for individuals
to relax and enjoy time with loved ones.
If needed, transition to home care hospice progresses smoothly,
with all necessary equipment and medications in place prior
to the patient leaving the unit. Home care nurses
work closely with the inpatient staff to assure that the
plan of care is continued. Patients on home care, regardless
of whether they are admitted directly to the service from
home or through the Inpatient Unit, always have the option
of returning to the Unit if further inpatient management
is needed.

- Medical Directors and attending physicians; many of the
medical directors are board certified in hospice and palliative
care
- Specially-trained nurses and certified hospice and palliative
care nurses
- Licensed and Master’s prepared Social Workers
- Spiritual care services to meet each individual’s
needs
- Certified home health aides
- Volunteers who provide support, companionship, and respite
care
- 24-hour a day, 7-day a week on call nursing staff available
by phone, with emergency nursing visits provided as needed
