- The Saint Barnabas Hospice and Palliative
Care Center and Van Dyke Hospice are licensed
by the State of New Jersey and are Medicare
and Medicaid Certified. Medicare- and Medicaid-eligible
patients are fully covered for care related
to a diagnosis of advanced illness.
- Saint Barnabas Hospice and Palliative
Care Center is a preferred provider for
most HMOs and managed care organizations
in New Jersey and is accredited by the
Joint Committee on Accreditation of Healthcare
Organizations (JCAHO).
- As affiliates of The Saint Barnabas Health
Care System, we are community-based and
at the same time are able to provide comprehensive
care throughout 10 New Jersey counties.
- The Saint Barnabas Hospice and Palliative
Care Center operates dedicated hospice
inpatient units at Monmouth Medical Center
and at Newark Beth Israel Medical Center.
Hospice Inpatient services
are also available at other Saint Barnabas
Health Care System hospitals and at select
skilled Nursing facilities.
- Comprehensive Hospice services are offered
at most nursing homes and assisted living
facilities within our service area.
You may ask your physician to approve
a hospice assessment visit. Effective management
of patient pain and other symptoms are
our primary goals once a patient is on
Hospice. By providing Hospice care, family
caregivers can actively participate in
the care of their loved ones, and find
support and guidance during these difficult
times. They can also be supported in many
ways during bereavement. Since most Hospice
related costs are covered by insurance,
the financial burden is often lifted from
our patients and their families. Hospice
is most effective when there is ample time
for assessment, development of plans of
care, and the strengthening of relationships
with patients, families and caregivers.
Our mission is to provide physical, emotional,
social and spiritual care to patients and
their families. To achieve this, we offer
a multitude of services handled by an interdisciplinary
team that is trained to provide expert
care to be of the utmost assistance at
this special time.
Specially trained Hospice nurses visit
the home or facility as often as necessary
to manage patient care and assess the needs,
make recommendations, teach care-giving
techniques, and provide support to the
caregivers or assistance to the family.
A Registered Nurse is on call 24 hours,
7 days per week to address symptom management
and other crises.

Social workers have advanced education
in counseling and helping individuals and
families cope with the emotional, social
and financial stresses associated with
life-limiting illness. They can also help
connect families with community services
when needed.

Hospice chaplains provide, upon request,
spiritual counseling and support for patients
and family members. Spiritual care is non-denominational
and does not impose any one religious or
philosophical belief, but can also support
one's specific religious denomination when
required. Ordained clergy and other spiritual
care providers offer guidance or consult
with the patient's own clergy. Memorial
services are held regularly and are open
to the public. Our chaplains can also contact
the family's own clergy to address patient
and family needs.

Specially-trained volunteers, offer
helping hands and their gift of listening skills
to meet the special needs of patients and families.
They can provide vital services such as companionship,
emotional support and respite care. Hospice
provides volunteer training and recognition.

For thirteen months following the loss
of a loved one, bereavement support is
offered to bereaved family members through
calls, referrals and specialized
bereavement support groups. Our groups
and consultations are open to the general

Additional specialists, such as physical,
occupational, nutritional and speech therapists
are available when necessary under the
direction of the Hospice team.
