

The Patient Access Department arranges for your room assignment on the day of admission. Most of our patient rooms are semi-private (two patients in one room).
A limited number of private rooms are available and can be requested one day prior to admission.

If you have had time to prepare for this hospitalization, you have already provided our staff with your health insurance information. However, if this is an urgent situation, a representative from the Patient Access Department will contact you or a family member to obtain this information. Please be sure the information you provide is accurate and includes all health plans under which you may be covered.
As part of the admission process you will receive a patient dentification arm bracelet. This bracelet is very important and will be checked by different members of the health care team before you receive any tests, treatments, procedures or medication. You should check the bracelet before it is applied to your wrist to make sure the personal information such as the spelling of your name, age and birth date are correct. Please do not remove your identification bracelet prior to being discharged from the hospital.
After the admission process is completed and your room is prepared, you will be escorted you to your room.
For your convenience, pre-registration is available for scheduled outpatient services and tests. To complete your registration by phone, please call 732.557.2550 ext. 12550 option 3. To avoid delay, call as soon as possible. Pre-registration can be done any time before your date of service. Please remember to bring your insurance referrals that may be required along with your insurance cards and identification.
You or a family member will be asked to provide insurance information, including insurance cards, pre-certification forms or other documents that your physician may require.
If you have a Living Will (Advance Directive) it is important to provide a copy so that your health care team knows your wishes and the name of the person you appointed as your Health Care Proxy. If you did not bring a copy with you, please ask a family member to bring your copy as soon as possible.
You will also be asked about the medications and dosages you take at home. If you already have your medications on a list, please bring that list with you (including all over-the-counter drugs, vitamins, herbal remedies and other supplements.)
