CASE MANAGEMENT / SOCIAL WORK SERVICES (973) 926-7278 In conjunction with medical and nursing services offered at the Medical Center, patients and families may need assistance in coping with the psychosocial aspects of illness. Services such as counseling, comprehensive discharge planning or placement in alternate care facilities, disease education, financial assistance, transportation, and referrals to federal, state and local agencies are available through Case Management/Social Work Services.
