Sub acute Care
Sub acute care is designed to return
patients to the community or transition them to a lower
level of care than a hospital.
This level of care is comprehensive
and cost-effective for patients who have been hospitalized
for treatment of an injury, illness or disease. The candidate
for sub acute care no longer requires the intensive procedures
of an acute care hospital, but does require the diagnostic
or invasive procedures of an inpatient health care facility.
The patient's care still requires active physician direction,
professional nursing care, significant ancillary services,
an outcomes-focused interdisciplinary approach to care,
and complex medical and/or rehabilitative care.
The Saint
Barnabas Nursing and Rehabilitation Centers are equipped
to handle every facet of post-hospital care including
medically complex and intensive rehabilitative cases.
These services are provided at a much lower cost than
those in acute care settings, lessening the financial
burden on families and hospitals. Cost-effective, outcome
oriented therapies ensure patients receive the most advanced
medical care.
The Centers'
progressive team of nursing and rehabilitation specialists
receives ongoing training and education to continually
upgrade their skills. They use innovative approaches to
provide the highest level of care available. Most importantly,
their understanding and personalized attention in a more
comfortable and welcoming environment contributes to a
speedier recovery.
The program
assists patients in regaining lost or diminished abilities
and restoring independence and confidence by focusing
on realistic, attainable goals. And since therapy patients
receive a continuum of care by working with the same healthcare
professionals each day, they achieve optimal results.
Additional onsite health services
include laboratory, radiology, podiatry, dental, ophthalmology
and counseling services.
