PELVIC RECONSTRUCTIVE SURGERY

Serving as a regional referral center for gynecologic oncology as well as reconstructive pelvic surgeries and gynecologic diagnostic dilemmas, the Division (through its clinical entity, Gynecologic Cancer and Pelvic Surgery, LLC) continues to attract patients throughout New Jersey and the surrounding tri-state area. Thad R. Denehy, M.D. and Robert R. Taylor, M.D. are board-certified in Gynecologic Oncology as well as Obstetrics and Gynecology and are nationally recognized gynecologic oncologists and pelvic surgeons.

Longtime partners James L. Breen, M.D. and Caterina A. Gregori M.D. recently retired in good health after serving as Chairman/Vice Chairman of the Department of Obstetrics-Gynecology at Saint Barnabas Medical Center for over 33 years. Both were named "Surgeons of the Year" by the Society of Gynecologic Surgeons and Dr. Breen was the President of the American College of Obstetricians and Gynecologists from 1984-85. Drs. Denehy and Taylor sorely miss their personal and professional association and strive to continue their tradition of surgical expertise and resident education.

Expert Treatment
Drs. Denehy and Taylor treat patients with conditions in three areas: gynecologic malignancies, pelvic reconstruction and complicated benign conditions. In addition, the Division fosters innovative clinical research in gynecology cancer through its research arm, Gynecologic Oncology Education and Research, P.C., to provide patients with the most advanced early detection and modern treatment options.

Gynecologic Oncology
The gynecologic oncology aspect of the Division provides a comprehensive range of services necessary for the proper treatment of gynecologic malignancies. These include highly specialized radical cancer surgeries, cytoreduction (reduction of the tumor mass through aggressive surgery), bowel and urologic surgery, lymphadenectomies (excision of the lymph nodes), implantable vascular ports, laser and laparoscopic surgeries as well as the latest in multi-agent chemotherapeutics in selected disease entities. The Division manages the full spectrum of gynecologic malignancies of vulvar, vaginal, cervical, endometrial, uterine, ovarian, and gestational trophoblastic (molar) origin. Physicians are experts in the diagnosis, management and treatment of the abnormal Pap smear as well as preinvasive disease of the cervix, vagina, and vulva. Consultations for hereditary gynecologic cancer syndromes are also available.

Reconstructive Pelvic Surgery
Another major function of the Division is the role of a regional referral center for reconstructive pelvic surgery. The entire spectrum of various pelvic support defects (hernias), such as posthysterectomy vaginal vault and uterine prolapses, cystoceles, rectoceles, or enteroceles (vaginal herniations of the bladder, rectum or small bowel), as well as urinary incontinence, are expertly and comprehensively treated. Through their over 20 year association with mentors Drs. Breen and Gregori, Drs. Denehy and Taylor have trained to accomplish their reconstructive surgeries without an abdominal incision, which has become crucial given the aging population. This affords a markedly lower complication rate. Additionally, through publications, lectures and practice, the Division has championed the resurgence of a minimally invasive procedure (LeFort colpocleisis) for the treatment of symptomatic severe pelvic organ prolapse in medically compromised elderly women. This year, the Division has introduced a new treatment (Transvaginal Tension-free Tape - Obturator) in its management of stress urinary incontinence. This procedure provides the same support and excellent outcome as the older procedure, Tranvaginal Tension-free Tape, but with a markedly less chance of serious complications.

Other Gynecologic Conditions
The Division also repairs extensive perineal lacerations occurring after childbirth that may be associated with incontinence of feces and/or urine as well as marked vaginal or pelvic relaxation. These procedures are also performed transvaginally, resulting in minimal discomfort and a shorter hospital stay. Urologic/rectal fistulas associated with incontinence of urine, stool, or flatus and their drastic effect on patients’ quality of life are diagnosed and managed. Chronically painful incisions with neuromas that may accompany perineal lacerations or may occur after hysterectomies are also addressed by Drs. Denehy and Taylor.

Many physicians and surgeons around the region refer their technically challenging, medically compromised and high risk patients to the Division for surgical and medical management of both benign and malignant gynecologic conditions. The Division prides itself in cautious medical conservativism with appropriate expeditious surgical intervention via a team approach with minimal morbidity in these medically compromised patients. The Division has expertise in the surgical management of severe endometriosis and other benign gynecologic conditions complicated by a history of multiple prior surgeries. In these situations, the surgery may be difficult due to the development of adhesions (abnormal scar tissue). The Division physicians employ a team approach in these difficult cases. Patients benefit from the shared surgical expertise that optimizes outcome.

Lastly, the Division is also a well-known national referral source for the management of young women with congenital anomalies of the vagina and uterus such as vaginal agenesis (women born without a vagina and/or uterus), vaginal septae and obstructed uteri.

Individualized Medical Care
The Division enjoys a close working relationship with the Departments of Radiation Oncology and medical oncologists as well as other highly regarded physicians and surgeons. Regular multidisciplinary conferences and tumor boards are held to insure patients receive state of the art care with keen attention to the individualization of their medical care. The Division also participates in ongoing clinical and basic science research with a current focus on determining the sensitivity of tumor cells to chemotherapy agents.
