When a person has advanced cardiac disease it affects the entire family.
However, with support that addresses both the illness and the unique
needs of the patient and family, everyone involved can experience a
higher quality of life.
This concept is the cornerstone of the Cardiac Special Care Program
offered by Van Dyke Hospice and Palliative Care Center.
The program is designed to help patients and families increase caregiver
confidence and patient comfort, manage symptoms
quickly and effectively and reduce emergency room visits and lengthy
hospitalizations.
A patient’s primary care physician or cardiologist can
help families determine when the time is right
to seek extra support when caring for a loved one
with advanced cardiac disease. Early referral to the program increases
its benefit to the patient and allows both the patient and family to
meet their wellness goals.
