The Geriatric Assessment Center provides primary care and consultation
services to patients age 65 and over. The program also provides comprehensive
evaluation for patients who exhibit the signs and symptoms of dementia
and/or have multiple chronic problems. The Geriatric Assessment Team
evaluates the patient's physical, social and psychosocial needs and
then provides recommendations to improve the client's quality of life.
The comprehensive medical evaluation would be important if any of the
following are demonstrated:
- Suffers from depressed or altered behavior
- Decline in functional abilities
- Confusion or memory problems
- Difficulty with mobility
- Problems preparing food, handling money
or taking medications
- Takes multiple medications and required recommendations
on present or new ones
- Requires coordination of services
Psychosocial evaluations are performed by a social worker to assist
the patient and family with homecare needs, educational resources and
counseling services.
The Geriatric Health Center accepts reimbursement from Medicare, Medicaid
and most private insurance plans. Families are advised to call and verify
insurance coverage.
