Our Cardiac Transfer Center offers one point of contact
to arrange for the transfer of any patient requiring advanced
levels of cardiac care. The physician who receives the patient
remains involved throughout the transfer to ensure continuity
of care.
- Expedite transfer from any hospital to the appropriate
SBHCS facility
- Facilitate referral to the preferred physician
- Secure a bed
- Schedule a procedure
- Arrange for ambulance or car transport as necessary
- Obtain insurance pre-certification
- Transfer all patient information
- Address family issues about transportation/housing as
appropriate
- Coordinate feedback on patient status
- 24 hours a day, 7 days a week
