That You Will Need To Know Regarding The Registration
Process
Registrations must received no less than (10) ten business
days prior to your selected choice date. This will
allow enough time to mail the required materials to each
student to prepare for their course.
Please print clearly to assure
proper spelling and complete all required
information on the registrations form.
All renewal class registrations must
submit current copy of Healthcare Provider card.
To be eligible for renewal, your current card must not
exceed the month’s end of the recommendation
date to renew.
ACLS initials must submit a current
copy of Healthcare Provider card along
with registrations form; please do
not submit separately. Also, if you are an employee
of SBHCS, please forward a copy of employee ID – in
order to be charged the correct rate.

current copy of ACLS card along with registration
form; please do not submit separately. Also,

PALS initials must submit current

Registrations will not be processed without a form of
payment provided.
We will accept faxed registration forms if
you are an employee of SBHCS and your Department Head has
approved the course by completing and signing the employee
section of the form; which is located at the bottom of
the registration form and fax to 973-923-6437.
We will accept faxed registration forms if
your payment method is via credit or debit card. Please
complete the mid-section of the form and fax to 973-923-6437.
All other payments made via checks and money order payments must
be submitted via mail or walk-in. The CPR Department
is located at Newark Beth Israel Medical Center; located
in building “N” - 4th floor all entrance
is through the 201 Lyons Avenue, Newark NJ. Office
hours of operation are Monday through Friday from 8:30
am – 4:00 pm (closed on holidays).
The Community Training Center will review the registration
form for completeness and you will be notified shortly
via mail regarding your course selection. Incomplete
registration forms will be returned to the address listed
on the registration form. It will result in an unconfirmed
class; and a course closure may occur during this delay.
Important Information
That You Will Need To Know Regarding Card Processing
At the end of a ACLS/PALS course,
we will provide you with a course completion letter that
will give you a result of “Congratulations” – “Re-Evaluation
Required” or “The Course Must
Be Repeated”, this letter also, serves as
proof for continuing education credits and it is valid
for 30 days from the course date.
At the end of a BLS course, you
will be provided with a course completion letter, this
letter serves as temporary proof of course completion and
it is only valid for 90 days from the course date. If
you have not received your card within six weeks, please
give the Community Training Center a call at 1-888-277-2281. You
have up to 90 days to report that your card has not been
received, after which, we will consider it as a card replacement.
There is a $10.00 card replacement fee if your card has
been damaged, lost or misplaced.
Please be advised that the cards are mailed to
each applicant at the address listed on the registration
form. It is the responsibility of the
participant/employee to provide a copy of
their current card to their Department Manager and Human
Resource department of their workplace.
